Protection of the Personal Information of Students, Staff, and Faculty Members by the Center for Health Sciences and Counseling

 The Act on the Protection of Personal Information entered into force on April 1, 2005. This Center has always taken great care in the handling of personal information. In future, we will continue to demonstrate meticulous care in the handling of the personal information of those who use our services, ensuring that it is used only for the purposes outlined below. We will also put in place a system to prevent leaks of personal information, by ensuring that physicians, counselors, public health nurses, other nurses, and all other staff who handle health information are aware of the importance of personal information and comply fully with their duty of confidentiality. If you have any feedback concerning the Center’s handling of personal information, please do not hesitate to contact us.

1. Purposes for which personal information is used by this Center

(a) Results of medical examinations

  • Provision of services at this Center, including medical care and health counseling
  • Issue of medical examination reports, medical examination certificates, and various other certification
  • Referrals to other medical institutions, responses to inquiries from medical institutions (only if the individual concerned has granted consent)
  • Disease prevention, health management (lifestyle-related diseases, overwork, etc.)
  • Management of the working environment, work management If data from an examination conducted by another medical institution are submitted (such as the results of a complete medical check-up), they are treated as medical examination data.

(b) Information obtained in the course of medical care and health counseling

  • Provision of services at this Center, including medical care, advice, and counseling
  • Referrals to other medical institutions, responses to inquiries from medical institutions
  • Communication with family members

 The consent of the individual concerned will be obtained each time the Center contacts another medical institution or communicates with the family of the individual concerned.

(c) Information gained in the course of health and safety management activities

  • Prevention of health problems, management of the working environment, work management

(d) Information that we are obligated to provide by law

  • Notifications that we are obligated to submit under the Industrial Safety and Health Act, the School Health and Safety Act, the Tuberculosis Prevention Act, and the Infectious Diseases Prevention Act, among others.

  • Basic data for maintaining and improving the services provided by this Center
  • Statistical information that does not identify the individual
  • Case reviews/studies aimed at improving the quality of medical care, health counseling, and other counseling (Adequate consideration will be given to the privacy of the individual in such cases and steps will be taken to ensure that they cannot be identified)

2. Written consent will be obtained if we wish to use your personal information for a purpose other than those listed above.

3. Upon completing the prescribed procedures, you may request or apply for the disclosure or amendment of personal information.
